Every entry is drawn from the Region X ALS SOP drug monographs and the protocols that reference them. Doses in the protocol section of the SOP always govern — when a monograph and a protocol differ, follow the protocol and contact OLMC if unclear.
Calculated volumes assume the concentration listed in the SOP monograph. Confirm the label on the vial you are holding before you draw — midazolam in particular is stocked in two concentrations. For ketamine, Region X doses off ideal body weight by height, not measured weight; the chart is in the ketamine card.
